Catalog description

This course explores the role of the professional nurse in care coordination to transform healthcare, initiating at the bedside and extending to transition in care. Relationships between health information technology; collaboration; care coordination; and evidence-based, patient-centered care are examined as central forces that impact health outcomes across populations and systems. Focus is placed on developing students? abilities to participate in care management of healthcare consumers in a variety of healthcare settings to maximize their independence and quality of life.
