Catalog description
This course focuses on person-centered care of adults with chronic illness within a Culture of Health framework. Palliative care is addressed with emphasis on an advocacy and shared decision-making model. Nursing considerations related to care coordination and care transitions are included. Application of clinical judgement occurs in simulation and clinical in chronic care and community settings.
