Nurse Practitioner Practicum: Clinical Synthesis

Catalog description
This course is the capstone in the preparation of Advanced Practice Registered Nursing students in the scope of a family nurse practitioner. Students will demonstrate an independent ability to assess, diagnose, treat, and manage persons of all ages with stable chronic conditions, acute episodic illnesses, and complex, co-morbid conditions commonly encountered in primary care settings. Students will display the ability to synthesize evidence-based practice guidelines and apply the evidence in the development of culturally appropriate and person-centered care plans as well as evaluate the outcome of those care plans. Students will continue to ensure the health promotion and health maintenance for persons of all ages. Prerequisite: NUR 62250. Prerequisite(s): NUR 62250 FOR LEVEL GR WITH MIN. GRADE OF D- Course Learning Outcomes 1. Manage clients with complex health problems by utilizing a multi-disciplinary approach in common primary care settings (Aligns with Program Outcome #1). 2. Synthesize evidence-based information in the development of detailed care plans, implementation of coordinated treatment strategies, and collaboration with multi-disciplinary teams (Aligns with EPSLO 1, 3). 3. Implement evidence-based guidelines and strategies to lead effective transitions of care and care coordination (Algins with EPSLO 2, 3). 4. Evaluate methods to monitor practice and make informed adjustments to improve patient care across the lifespan (Aligns with EPSLO 1). 5. Demonstrate accountability to practice within the regulatory standard and scope of a family nurse practitioner (Aligns with EPSLO 6). 6. Communicate the scope, responsibilities and contributions of a family nurse practitioner in various professional and public settings (Aligns with EPSLO 6). 7. Conduct patient assessments, develop and implement care plans, and make clinical decisions without direct supervision (Aligns with EPSLO 1, 6). View Class Schedule
