This course prepares Family Nurse Practitioner students to provide advanced, evidence-based, and culturally appropriate care to aging individuals and families. Students will focus on managing uncomplicated chronic conditions and acute episodic illnesses in primary care settings, emphasizing critical thinking, diagnostic reasoning, and the application of evidence-based knowledge to develop comprehensive, person-centered care plans. Students will develop competence in integrating health promotion and maintenance strategies into primary care for adults, older individuals, and their families. Prerequisite: NUR 51101. Prerequisite(s): NUR 51101 FOR LEVEL GR WITH MIN. GRADE OF D- Course Learning Outcomes 1. Integrate advanced knowledge, critical thinking, and clinical judgment to formulate differential diagnoses for adult and older adult persons (Aligns with EPSLO 1). 2. Analyze the pathophysiological changes and management implications for the most common uncomplicated chronic and acute disease processes in the adult and older adult populations (Aligns with EPSLO 1). 3. Evaluate evidence-based pharmacological and non-pharmacological treatments for common health issues encountered by adult and older adult populations (Aligns with EPSLO 1). 4. Develop a comprehensive, cost-effective care plan for adult and older adults with acute and uncomplicated chronic conditions, addressing social determinants to promote health equity (Aligns with EPSLO 1, 2). 5. Design patient-centered management plans by applying scientific principles and adhering to evidence-based practices, aiming to optimize adult and older adult patient outcomes (Aligns with EPSLO 1, 2).
