Care Coordination and Transition in Care

Catalog description

This course will address the care coordination and transitions in care when caring for patients. Emphasis will be placed on the needs of patients with chronic diseases to include mental health and substance use conditions with a focus on population outcomes. Prereq: Enrollment in Primary Care Workforce Development Certificate (Category 1b), or Registered Nurses (RNs) in Primary Care Workforce Development Certificate (Category 4), or RN to BSN program, or BSN program, or permission of instructor.
