Person Centered Care for Advanced Nursing Practice

Catalog description

This course focuses on developing advanced competencies in care coordination, transitions of care and communication to optimize patient outcomes. Using empathy, compassion and holistic approaches, students will learn to identify gaps in care, synthesize outcome data for evidence based practice and foster community partnerships to support self care management. The course prepares students to lead interprofessional teams and deliver person centered care within the full scope of nursing practice. Offered: Fall, Spring, Summer.
