1-5 This course applies principles of clinical prevention and population health to the practice of care management, including assessing, planning, intervening, monitoring, and evaluating health care delivery. Emphasis is placed on evidence-based, coordinated, and cost-effective approaches to care for rural and medically underserved populations. Interdisciplinary collaboration, integration of community and clinical resources, and evaluation of outcomes related to quality, access, equity, and cost are central components.
