Rural Population Health

Catalog description

3-1 This course explores the principles and practice of care management within the framework of clinical prevention and population health. Emphasis is placed on the integration of evidence-based models to promote health, prevent disease, and manage chronic conditions across diverse populations, with a focus on rural and medically underserved communities. Students will examine the evolving role of the care manager as a leader in coordinating care across the continuum, addressing social determinants of health, and fostering interprofessional collaboration. Key concepts include assessing population health needs, designing and evaluating interventions, and measuring outcomes related to access, quality, cost, equity, and client satisfaction.
