Chronic Transitional Care

Catalog description
This course explores professional nursing practice and its role in health promotion and injury prevention in adults with chronic health conditions in a variety of settings. Nursing, behavioral, and physical sciences are integrated throughout to support the application of transitional nursing care. Using a holistic care approach, the chronic health needs of patients are considered and evidence-based care plans are implemented. Key concepts of nursing practice that have been fostered throughout the nursing core are applied to complex patient care situations to challenge clinical reasoning skills. Evaluating patient care, fostering nursing professionalism, and collaborating with healthcare teams are emphasized.
