Continuum of Care Across the Lifespan

Catalog description
This pilot course will focus on the care and education of outpatient children, adolescents, adults, and older adults living with chronic, comorbid conditions. Emphasis will be placed on conditions resulting in frequent inpatient hospital stays and those which can be managed on an outpatient basis provided necessary resources and education. Opportunities will be provided for students enrolled in either/both nursing and/or Public Health to better understand reasons for and work toward developing tools/education that could decrease increased hospitalization rates secondary to poor disease management and decrease inpatient length-of-stay and readmission rates for chronic health care disparities that are relevant to the Pittsburgh area. The community focus will highlight the physiological, psychological, emotional, and socioeconomic implications of care of individuals across the lifespan. Students will complete a mentored leadership project focused on holistic considerations from learned didactic and community-based learning.
