Catalog description

Provides an in-depth review of evidence-based transitions of care models that demonstrate readmission risk mitigations strategies, patient engagement and a focus on goal of understanding population health management. Emphasis is placed on understanding adult learning theory and related theories on self-care. Students will collaborate to manage clients across the life span in their area of clinical expertise. The minimum requirement for this practicum is 180 hours. Enrollment Restrictions: Enrollment limited to students admitted to the Clinical Nurse Specialist track. Enrollment Requirement(s): NURS 534T must be taken concurrently. Prerequisite(s): NURS 532T and NURS 533T-A or NURS 533T-B . Corequisite(s): NURS 534T .
