PHRM 580L: Special Topics in Family Medicine. This elective course is designed to build upon and supplement knowledge of common, long-term illnesses encountered in ambulatory and hospital family medicine practice. Students will actively engage in various learning activities to apply the Pharmacists’ Patient Care Process to advanced pharmacotherapeutic inpatient and outpatient management of disorders which may include (but not limited to) diabetes, anticoagulation, atrial fibrillation, hypertension, heart failure, polypharmacy, COPD or asthma, pain, acute kidney injury, and chronic kidney disease. The focus will be on holistic care, transitions of care, and continuity across the family life cycle. In addition, students will explore ambulatory and institutional pharmacist patient care models and pharmacist provider status. Students will develop tools and practice skills to improve success on their APPEs. (
