APPE II: Ambulatory Care

PHRM 573: Advanced Pharmacy Practice Experience II: Ambulatory Care Pharmacy This Advanced Pharmacy Practice Experience (APPE) builds on the knowledge and skills students have acquired during their P1 - P3 years through direct patient care in an ambulatory care practice setting, with a focus on provision of care for patients with Ambulatory Care-Sensitive Conditions (ACSCs). Key goals of all APPEs are to continue the development of students’ critical thinking and problem-solving skills through hands-on experiences. During this experience, students will actively take part in many ambulatory care pharmacy practice activities/experiences including: individualized patient counseling; ongoing drug therapy monitoring; medication therapy management as appropriate; patient education material development; and active collaboration with other healthcare professionals in the drug therapy decision-making process. The goals of this APPE are to provide students with opportunities to: (1) develop knowledge and clinical skills in the provision of patient-centered care at the individual and ACSCs patient population level; (2) build clinical decision-making skills and confidence in these skills; (3) develop safe, cost-effective and individualized patient care plans which may be documented in the patient’s medical record and implemented through communication with the patient and providers; and (4) monitor care plans through collection and interpretation of patient data including patient interviews.
