PHRM 545: SKILLS/ SMALL GROUP COMPLEX CASES: Integrated Pharmacotherapy I - IX: Whole System Overview Integrating Concepts and Advanced Patient Care In preparation for progression into Advanced Pharmacy Practice Experiences (APPEs), the focus of this course is to help students integrate and apply prior learning from previous courses to formulate clinical solutions and care plan recommendations for treatment and management of diseases and disorders in patients with multiple comorbidities. Students will have hands-on opportunities to identify, assess and resolve disease-related problems in patients with multiple comorbidities through using a series of comprehensive cases based on real-world clinical cases; students will be guided and expected to apply their knowledge and critical thinking skills to recommend real life clinical solutions. Comprehensive patient documentation will be used in students’ preparation for patient case presentations and as a means of documenting key discussion points. Students will reinforce key clinical concepts and acquire skills in developing management and treatment plans for complex patient cases as final preparation for their progression into APPEs where they will encounter correspondingly complex patients. Additionally, students will participate in an IPE activity/simulation. (
