Applies higher-level critical thinking, teamwork, effective communication, informatics and design process required to develop a 'real-world' health care coordination solution that will be a catalyst for change by improving safe patient-centered transitions, lowering readmission rates, increasing patient safety, and/or decreasing the cost of care.
